Care Manager/RN

Responsibilities:

• The care manager will be responsible for sitting in the Physician practice and handling about 30-45 calls per day (outbound).
• The CM will be responsible for handling 80-100 cases at a given time.
• The CM will be responsible for updating care plans over the phone and working with clinicians to provide the next best steps for post discharge care.
• The role is 95% telephonic and 5% in person. They will be working with 10 physicians

Requirements:

•  RN license       
• 1 year CM experience         
• Fast learner      
• Strong communication and technical skills 
• EPIC experience     
• CM Certification
